Job Description

Squirrel Hill Health Center (SHHC), is a Federally Qualified Health Center (FQHC), seeking a Patient Advocate for our Primary Care Team. The Care Team Patient Advocate will collaborate with primary care team members to support high-quality, well-coordinated, patient-centered care for patients and their families/caregivers/support systems. ​ The Patient Advocate will support the assigned primary care teams in various capacities. ​

Essential Support Functions:

  • Referral Management: Review referral orders, ensure necessary documentation, and manage closed loop referrals. ​
  • Order Tracking & Follow Up: Track and follow up on specialty referrals, imaging, and other orders. ​
  • Prior Authorizations: Communicate with payers/health plans and external agencies for timely prior authorization processes. ​
  • Patient Scheduling & Reminders: Assist with patient scheduling and communication to promote continuity of care. ​
  • Patient Communication & Engagement: Interact with patients to understand and address needs, identify SDOH risk factors, and connect patients to appropriate services. ​
  • Care Gap Closure: Promote closure of Care Gaps through targeted communication campaigns and patient engagement strategies. ​
  • Data Collection & Reporting: Provide routine data and reporting to demonstrate performance, quality, and value. ​
  • Care Team Support: Collaborate with care team members and maintain accurate documentation in EMR and other HIT systems. ​
  • Professional Relationships: Establish and maintain effective working relationships with internal and external customers. ​
  • On-site Work: Required to work on-site when necessary, as determined by the supervisor. ​
  • Other Duties: Perform other duties as assigned. ​

  • Collaborate with practice providers, clinical staff, ancillary care teams, patients, and families/caregivers. ​
  • Engage others and model positive behaviors of team-based care philosophy. ​

Qualifications / Position Requirements:

  • Preferred:
    • Previous healthcare experience ​
    • Experience with electronic scheduling systems ​
    • Experience with electronic medical record systems ​
    • Familiarity with basic medical terminology ​
    • Basic knowledge of healthcare insurance (benefits, authorizations, and referrals) ​
    • Basic knowledge of healthcare-related data and reporting ​
    • Bilingual (Spanish/English) ​
  • Required:
    • All CDC recommended immunizations, including updated COVID-19 vaccines ​
    • Must be 18 years or older ​
    • High School Diploma or equivalent ​
    • Ability to perform routine assignments independently ​
    • Attention to detail and organizational skills ​
    • Excellent verbal and written communication skills
