The Primary Children's Center for Safe and Healthy Families (SHF) provides medical and behavioral health services to victims of child abuse and their families. The SHF Medical Social Workers are part of a multidisciplinary medical team of Child Abuse Pediatricians, Forensic Nurses, Child Life Specialists, other child abuse-trained medical professionals. The medical team responds to the emergency department and inpatient units at Primary Children’s Hospital when asked to consult on patients where the causes of injuries or illnesses is unclear and there are concerns for abuse. The Medical Social Worker’s role during consults is to provide emotional support to patients and families, assess for family needs or safety risks in the home, and provide the family with appropriate resources and referrals. The medical team routinely communicates with Child Protective Services, law enforcement, other healthcare providers, and other community agencies to provide timely care for the patient and their family. The behavioral health trauma treatment team at Safe and Healthy Families consists of psychologists, clinical social workers and psychiatrists who provide evidence-based trauma treatment to assist children and their families who are experiencing emotional and/or behavioral problems related to trauma. The Medical Social Worker supports the trauma treatment team by screening therapy referrals received by phone, email or fax to determine whether a child might benefit from the therapeutic modalities our team offers and by conducting behavioral health phone intakes when needed. Job Specifics Pay Range Clinical: $35.25 - $54.39 Non Exempt Pay Range Certified: $32.02-$49.44 Non Exempt Benefits Eligible: Not Eligible FTE: PRN Shift: Monday- Friday day time hours as needed Scope: This position will include shifts at both the Primary Children’s Outpatient Services Eccles Building in Salt Lake City and the Primary Children’s Hospital Miller Family Campus in Lehi. This position provides as-needed coverage for full-time staff to attend conferences, trainings, or use vacation time. The position requires availability for at least one eight-hour shift a week with flexibility to cover additional shifts as needed (Monday-Friday 9:00am-5:00pm). The ideal candidate will have availability to flexibly cover different days of the week and to increase hours based on team needs. Job Essentials: Organizes and prioritizes daily work by screening and assessing new, current, and discharging patient needs in area(s) of responsibility. Completes documentation as required. Performs social work consultation for patients and caregivers. Conducts psychosocial assessments. Provides supportive, crises, trauma support, bereavement support, and other social work interventions as necessary. Educates, communicates, and coordinates with multiple patient care teams and outside agencies, as necessary, to ensure the patient receives quality care, without delays, and to identify and resolve delays and issues. Screens and conducts assessments when abuse, neglect, exploitation, or legal issues are suspected or identified. Consults with protective service agencies (i.e. DCFS; APS; Law enforcement) to ensure patient safety, appropriate and timely interventions, and a safe transition to the next level of care. Provides support and education to patients, families and staff members to facilitate in advance care planning (i.e. advance directives, referrals to palliative/supportive and/or hospice care). Serves as the mediator/broker between patients, families and the medical team, as needed. Consults with clinical, administrative, and support services to address and/or reduce quality, legal, and other concerns and promote patient and family-centered care. Completes timely and accurate documentation in the medical record, using knowledge of documentation standards for the department, to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing/reimbursement standards. May coordinate the discharge planning process from patient admission to departure. Screens patients and assesses transition planning needs. Contributes to the patient's interdisciplinary plan of care. Coordinates and refers to internal and external resources for needed services to facilitate a safe and timely transition to the next level of care. Conduct risky behavior screenings for children ages 12 and up. Provide appropriate resources for identified needs. Administer suicide screenings for children ages 7 and up. Refer for immediate crisis evaluation in the emergency department, if needed, or provide appropriate mental health referrals as necessary. Provide crisis intervention, as well as appropriate referrals to mental health treatment or other resources as needed. Minimum Qualifications Master's Degree in Social Work. Education must be obtained from an accredited institution. Degree will be verified. Current State Licensure as LCSW, CMHC or CSW. Computer Skills, i.e., email, word processing. Excellent written and verbal communication skills. Preferred Qualifications Experience working within a healthcare setting. Experience working with Child Protective Services and/or the Division of Child & Family Services. Experience providing crisis intervention services. Experience working directly with children, teens, or families.
