Job Title: Home Health and Hospice Nurse Educator (Hybrid Role)

Location: Hybrid - Raleigh and Chapel Hill, NC + Remote capabilities

Job Description: We are seeking a skilled and compassionate Home Health and Hospice Nurse Educator to join our team in a hybrid role. This position is ideal for a Registered Nurse with strong OASIS assessment and home health experience, who is passionate about teaching, supporting patient care teams, and providing hands-on training and assistance as needed. The Nurse Educator will be responsible for educating staff, assisting with patient escalations, and providing in-home training and support for care teams. This role requires flexibility and a willingness to go wherever the need arises, including traveling to patient homes when necessary.

Key Responsibilities:

• Staff Education & Training : Educate and mentor physicians, staff, and home health professionals to ensure the delivery of high-quality, compassionate care. Provide hands-on training and support for clinical staff in both office and home settings.
• OASIS Assessment : Conduct OASIS assessments and ensure compliance with regulatory requirements. Train staff in completing thorough, accurate OASIS documentation.
• Patient Support & Escalations : Provide on-site assistance in patient homes when needed, especially during patient escalations, to ensure appropriate interventions are in place.
• Regulatory Compliance : Educate and ensure adherence to state and federal regulations related to home health and hospice care. Act as a resource for staff and referral sources regarding regulatory requirements and best practices.
• Flexible Role : Willingness to travel to Raleigh and Chapel hill office locaitons, patient homes as needed to train, assist with duties, and support care team efforts.

Required Qualifications:

• Education : Graduation from a state-accredited school of professional nursing (BSN preferred).
• Licensure : Must be a licensed Registered Nurse (RN) in the state of North Carolina.
• Experience : A minimum of 2 years of home health, hospice, or public health experience. Strong background in OASIS assessments is required.
• Skills : Excellent communication, teaching, and clinical skills. Ability to work independently and as part of a team. Strong organizational and problem-solving abilities.

Work Schedule:

• Standard 40 hours per week, typically 5-8 hour shifts. Consideration will be given for 4-10 hour shifts.
