As a Home Health Aide (HHA) the work you do every day makes a difference in the lives of our patients. Our clinicians give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment. As a Home Health Aide (HHA) you are an essential member of the home care team who works under the supervision of a nurse. You provide compassionate support with activities of daily living and basic personal care needs while ensuring a safe, comfortable living environment. Interacting with patients' families while caring for your patients and experiencing the rewarding privilege to be part of every step of their recovery journey. You'll perform personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene. You will effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team. You'll be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel. Major areas of responsibility include patient care (assist patients as needed with lifting and transferring), communication (maintain good relationships with the patient, the patient’s family, and staff, and report any changes to the patient’s medical and living conditions to the supervisor or case manager), minimizing patient risk (ensure the living environment is healthy and safe), policies (complete all documentation following agency protocol and Medicare/Federal guidelines), and operations (attend in-service training and mandatory agency meetings). Requirements include a high school diploma or equivalent, State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA), one year of experience as a nurse aide in a hospital, nursing home, home care agency, or private setting under direct supervision of an RN, completion of skills competency test with 80% or higher score, valid driver's license and auto insurance, and capability of all physical demands.
