The Community Health Worker (CHW) serves as a bridge between the healthcare system, social services, and the community, aiming to improve health outcomes, reduce barriers to care, and provide advocacy and support. The CHW will screen patients for and help address Health-Related Social Needs (HRSNs) and promote patient education, health literacy, and self-sufficiency.
