Field-Based Community Health Worker (Bronx & Westchester - Full-Time)
Position Overview:
We are seeking a compassionate and detail-oriented Care Management Coordinator to join a dedicated team serving members across the Bronx and Westchester County. This full-time, field-based role involves visiting members in their homes to complete brief wellness assessments, ensure appropriate services are in place, and support overall quality of care. This is an excellent opportunity for someone with a background in managed long-term care (MLTC), home care, or health plan operations.
Key Responsibilities:
Conduct home visits to approximately 10 members per day, primarily in Westchester County
Complete brief, structured in-home assessments (approx. 10 minutes per visit)
Observe home environment (e.g., medication supply, food availability, general safety)
Accurately document findings and enter data using electronic medical record systems
Communicate effectively with members, caregivers, and interdisciplinary plan teams
Monitor and report changes in member status or home environment to clinical staff
Maintain timely scheduling and submission of visit logs and mileage reimbursements
Attend office-based planning meetings on Mondays (Bronx location)
Field visits conducted Tuesday through Friday
Schedule:
Monday-Friday, 9:00 AM - 5:00 PM
Monday: In-office (Bronx) for scheduling and administrative tasks
Tuesday-Friday: Field-based home visits
Qualifications:
High school diploma or equivalent required; BA/BS preferred
Valid NYS driver's license and access to a reliable vehicle (mileage reimbursed)
2-3 years of experience in a managed care or home care setting
Familiarity with MLTC plans strongly preferred
Strong communication and customer service skills
Proficient with Microsoft Office and electronic documentation
Able to read, write, and speak English fluently; Spanish strongly preferred
Friendly, empathetic, and professional demeanor-especially when working with elderly or mentally vulnerable members
Ideal Candidate:
Has prior experience in the home care industry or working with health plan vendors
Feels confident conducting sensitive conversations over the phone and in person
Can navigate cultural, developmental, and emotional needs of elderly populations
Is organized, self-directed, and comfortable working independently in the field
