Program Summary:
Critical Time Intervention (CTI) is a trauma-informed, blended case management service that provides intensive support to adults experiencing homelessness in Montgomery County who also have significant mental health needs. CTI’s primary initiative is to locate and connect people to adequate, affordable housing units, and provide support as a client transitions from a shelter or other setting to a new residence. CTI can support clients with linkages to mental health and substance use services, collaborate with community providers, and assist in the development of a client’s natural support system. During this critical time, CTI monitors and assesses these supports to ensure their effectiveness in maintaining their new residence and breaking the cycle of homelessness.

Position Summary:
The Full-Time Case Manager is responsible for providing hands-on case management services to consumers in the field. The Case Manager will assist consumers in creating linkages to long-term community support with particular attention to five areas that facilitate community stability: psychiatric treatment and medication compliance, money management, substance abuse management, housing crisis management, and family interventions. The primary goals of the CTI Case Manager are to maintain engagement with the CTI consumer, and, to sustain the CTI consumer in a residential setting during the duration of the CTI service. This is essential in order to provide the stability and connectivity necessary to reduce future homeless episodes.

An Ideal Candidate Will Have:

  • Bachelor's Degree In Human Services field.
  • Valid Driver's License.
  • Be able to commute throughout Montgomery County.
  • Pass all required background checks and clearances.
  • Be willing to work in a homeless shelter, within the community and travel into the community with clients for appointments and meetings.

Responsibilities/Duties:

  • Participate in case management meetings at area shelters, client homes, and other sites within the community.
  • Participate in individual and team supervision as required by the Director.
  • Accompany consumers to appointments and meetings in the community.
  • Gain access to services with creative attempts to help consumers obtain resources that are needed. This includes a strong role as a consumer advocate.
  • Monitor service delivery by maintaining regular contact with consumers and service providers/programs involved with the client.
  • Create linkages to formal and informal supports to strengthen the client’s ties within the community.
  • Monitor consumers on a regular basis as determined by the consumer’s Phase of service to note any changes that may impact his/her treatment/recovery.
  • Maintain clear and concise documentation of all client contacts and audit consumer charts as required to ensure daily and quarterly forms are completed accurately and filed in the consumer’s chart.
  • Complete all required documentation for each consumer as required by the program and its funders.
  • Maintain weekly billing productivity requirements as established by the Director.

Required Experience, Qualifications, and Education:

  • A Bachelor's degree in social work, psychology, counseling, social work, education, or other social service-related field.
  • A valid PA Driver’s License is required.
  • Experience working with a homeless population and providing outreach to homeless individuals is highly desired.
  • Experience in working within the mental health field is also preferred.
  • Experience working with Recovery Model and Trauma-Informed practices.
  • Experience and knowledge using Motivational Interviewing and Stages of Change.
  • Proficiency with Microsoft Office Suite.
  • Must be able to meet with clients in crisis from anywhere within Montgomery County within 1.25 hours of their initial contact when working on-call.
  • The ability to interact professionally with clients, their family members, and other service providers using a trauma-informed approach is essential.
  • Must be able to work with a team and demonstrate flexibility.
  • Must be willing to work in a homeless shelter, within the community, and travel into the community with clients for appointments and meetings.
