Summary/Objective
Primary job responsibilities include supervising and managing Care Coordination staff providing care coordination and behavioral health services at Boundless Behavioral Health. Working with clients, parents/guardians, staff, and community partners, the Care Coordination Manager facilitates access to care and addresses barriers. The Manager will provide counseling, support, care coordination, and linkage both within Boundless service lines and with outside providers in order to promote meaningful treatment and improve outcomes for the individuals served. This position reports to the Director of Outpatient and Community-Based Services and works closely with all other departments. This is a full-time, salaried position.Essential Functions

Work Environment:
This job regularly works in the community (Delaware, Knox, Licking, and Morrow Counties), providing services outside of the office, in schools, client homes, and other community settings. This role routinely uses standard office equipment such as computers, phones, and photocopiers. (This job is hybrid - 85%/15% remote and in the community.)

Position Type/Expected Hours of Work:
This is a full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand.
Travel:
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion. This position does require regular use of a personal vehicle for local travel to meetings and appointments in the community.
